Terms and Conditions

1. Patients will be entitled to one (1) lucky draw entry on the purchase of Zia Medical
Center’s Back2School Pediatric Packages (on cash basis) or booking a Consultation
appointment (on medical insurance basis)
2. The Promotion is open from 24 Aug 2022 to 29 Sept 2022, both dates inclusive (the
“Promotion Period”). The lucky draw will be held at Zia Medical Center, Jumeirah Beach
Road on 30 Sept 2022.
3. The ZM number of the patient will be counted as the entry into the promotion. The
signed participation form is required for entry into the Promotion.
4. All details and particulars submitted in the participation form must be accurate, true and
complete. Incomplete, inaccurate or illegible forms will be disqualified.
5. Lucky draw entries after the stipulated promotion period shall be considered invalid and
void.
6. Each winner is only entitled to win one prize, regardless of how many times their ZM
number is picked. Zia Medical Center shall be entitled to disregard the second and
subsequent picks and re-pick a winner.
7. Zia Medical Center shall contact the Winners via phone within seven (7) days of the
conduct of the Lucky Draw (“Notification”).
8. The Winners are required to redeem their prizes within 14 days from the Notification,
failing which, the winner will not be entitled to the prizes.
9. The prizes are not transferable, exchangeable for cash or any other items.
10. Acceptance of any prize shall constitute consent on the winner’s part to allow the use of
the winner’s name, photo, image, voice and/or likeness by Zia Medical Center for
editorial, advertising, promotional, marketing and/or other purposes without further
compensation except where prohibited by law.
11. Employees of Zia Medical Center, their immediate family members, advertising
agencies, and any other parties directly involved in organizing, promoting, or conducting
the Promotion are not eligible to participate in the Promotion.
12. Zia Medical Center shall not under any circumstances whatsoever be liable for any loss,
injury, damage or delay arising out of any cause whatsoever beyond Zia Medical
Center’s reasonable control or any malfunctioning of or defect in or failure of any of the
prize or any loss of the use of any prize.
13. By entering the Lucky Draw or accepting the prizes, You agree that no claim relating to
such losses or injuries (including special, indirect and consequential losses) shall be
asserted against Zia Medical Center, its parent companies, affiliates, directors, officers,
employees or agents from any and all losses, damages, rights, claims and actions of any
kind resulting from the Lucky Draw and acceptance of any prize, including without
limitation, personal injuries, death and property damage.
14. Zia Medical Center has the sole discretion to exclude any person from participating in
the Promotion without any obligation to furnish any notice and/or reason.
15. By entering the Lucky Draw, you acknowledge that you have read, understand and
agree to Zia Medical Center’s Data Protection Policy as per.
